The YorWellbeing health checker is designed by City of York Council to calculate your risk of developing diabetes and cardiovascular disease in the next ten years.
The health checker will help you understand your risk, and will give you information about how you can protect your health. Depending on your results, you may also be invited for a personalised health check appointment.
You will be asked to provide some information about your current health. For example, health conditions you have or medicines you take. You will also be asked a small number of questions about your family medical history. We will also ask some questions about you, such as your name, age, and postcode.
Unfortunately this online health checker tool will not work if you already have diabetes, heart disease or another form of CVD. You may still find it helpful to use the links at the bottom of the website home page.
Most of the questions are easy to answer. If you do not know your current weight, it may be helpful to weigh yourself before starting. Also, if you have recently been told your blood pressure or cholesterol ratio it would be helpful to find your scores. However, this is not essential. You can complete the questionnaire without knowing your blood pressure or cholesterol ratio, do not worry if you do not know your scores.
This questionnaire is very short, most people complete it in 5 – 10 minutes.
This questionnaire will explain your risk of developing diabetes or cardiovascular disease within the next ten years. You will be offered some information about ways to keep your risk low and to protect your health. Depending on your results, you may also be invited for a personalised health check appointment. If this happens you will be given some information about how to book an appointment.
We will store your information securely. If you attend a health check appointment we will share your information with your GP. We will share your anonymised data with our partner university who is supporting us to evaluate the service. To find out more, please read our privacy notice.
